How do I fill out the FP-2506 form?

First, locate the FP-2506 file at the bottom of the Financial Information section. Then click on the pencil on the right to fill out the form, attach a digital signature and submit it. You can also download the form and save it for your files. Once the form has been signed by AXSO, it will be available under the Documents tab in the portal, where you will be able to download and save it.
